I was curious about what a premium longevity/wellness clinic would actually charge for a fairly advanced optimization protocol versus managing it yourself.

So I created a prompt and asked ChatGPT to estimate it based on my wife’s and my current stack and monitoring approach. Needless to say, it surprised me.

ChatGPT estimated that a comparable protocol through a premium longevity or concierge wellness clinic could realistically run anywhere from roughly $3,500–8,000/month combined depending on the clinic, markup structure, physician oversight, IV therapies, and concierge level.

The biggest cost drivers appeared to be:
    GLP-1s / Retatrutide
    Tesamorelin
    SubQ NAD+
    MOTS-c
    concierge program fees

I was honestly just curious what the equivalent clinic pricing would look like compared to self-managing.

If you’re curious too, here’s the prompt I used:

Based on what you know about our current peptide stack, help me estimate what a premium longevity / wellness clinic would charge monthly for a health optimization protocol if I were not self-managing it. Ask clarifying questions if you need to before you give estimate.

    Estimate:
    Monthly clinic pricing
Which compounds/services are typically marked up the most?
What is commonly included in concierge pricing?
The difference between DIY/self-managed costs versus clinic-managed costs.

    Factor in:
    Injection frequency
    Lab monitoring
    Concierge physician oversight
    Telehealth versus premium in-person clinics

    Pretty eye opening. Have fun!

      Copilot Response:

A premium longevity clinic would turn your current self‑managed peptide rhythm into a $1,200–$3,500/month subscription, depending on whether it’s telehealth‑only or a boutique in‑person program with white‑glove oversight. Below is the clean breakdown you’re looking for — short, direct, no fluff.
      Monthly clinic pricing (realistic ranges)

      Telehealth longevity clinics — $300–$900/month
Basic labs, generic peptides, minimal physician interaction.

      Mid‑tier concierge programs — $1,000–$1,800/month
More frequent labs, higher‑touch oversight, name‑brand peptides, structured protocols.

      Premium in‑person longevity clinics — $2,000–$3,500/month
Full concierge access, unlimited visits, advanced diagnostics, bundled peptides, IV therapies, recovery tech.

Your current stack (NAD+, AOD, DSIP, plus the occasional add‑ons you’ve explored) would place you in the $1,500–$2,800/month tier at most clinics.
Which compounds/services get marked up the most

NAD+ injections — clinics charge $200–$600/week for something that costs pennies to compound.

      Peptides like DSIP, GHK‑Cu, MOTS‑C, 5‑Amino‑1MQ — 3–6× markup over pharmacy cost.

IV therapies — especially NAD+ IVs ($400–$1,000 per drip).

Hormone optimization — bundled into “membership” even if doses are tiny.

Lab panels — $300–$1,200 per draw for panels that cost $80–$200 cash‑pay.

The rule: anything injectable or IV is marked up the most, followed by labs.
      What concierge pricing usually includes

Quarterly lab panels (CBC, CMP, lipids, hormones, inflammation markers)

      Protocol design + physician oversight

      Telehealth access or in‑person visits

      Peptide fulfillment (usually 2–5 peptides/month)

      Lifestyle coaching (nutrition, sleep, training)

Recovery tech access (sauna, cold plunge, PEMF, red light)

      Priority scheduling

      Discounted IV drips (still overpriced)

Premium clinics add: DEXA, VO2 max, microbiome testing, epigenetic clocks, continuous monitoring.
      DIY vs clinic‑managed cost difference
DIY / self‑managed (your current style)

      Peptides: $150–$350/month

      Supplies: $20–$40/month

Labs (cash pay): $80–$200 per panel

      Total: ~$250–$450/month

      Clinic‑managed equivalent

      Peptides: $600–$1,500/month (markup)

      Labs: $300–$1,200/quarter

      Membership fee: $300–$1,000/month

      Total: $1,200–$3,500/month

Premium clinics cost 4–8× more than DIY, mostly due to markup and bundled services you may not need.
      Injection frequency impact

Daily or near‑daily injectables (AOD, DSIP, GHK‑Cu, CJC/Ipam)
Clinics charge more because they bundle “monthly peptide supply” at inflated rates.

      Weekly injectables (NAD+, BPC‑157, MOTS‑C)
Often billed as “premium peptides” with the highest markup.

      IV therapies
The biggest profit center — especially NAD+ IVs.

      Telehealth vs premium in‑person clinics
      Telehealth

      Lower overhead

      Fewer diagnostics

      Peptides shipped directly

      Less frequent labs

      Cost: $300–$900/month

      Premium in‑person

      Full diagnostic suite

      Recovery tech

      In‑house pharmacy

      Unlimited visits

      Cost: $2,000–$3,500/month

      Bottom line

If you walked into a premium longevity clinic with your current stack, they’d package it as:

“Comprehensive Metabolic & Regenerative Optimization Program” — $1,800–$2,800/month

You’re currently running the same protocol for under $400/month.

If you want, I can map your exact stack into a clinic‑style pricing sheet or build a DIY vs concierge comparison tailored to your regimen.

These numbers are incredibly low. I talked to one clinic with a large online presence and they were $1,360/month just for Glow. Tirz was another $1,000. By the time you add in TRT/HRT and other stuff, you are going to be at least $5,000/month.

My entire annual program costs less than that.

100% Natty. You know, aside from the Klow, Tesa, Tirz, CJC/Ipa, SS-31, Semax, Dihexa, Epitalon, MOTS-C, creatine, Cheetos, protein shakes, and a holocaust of chicken thighs.
